Shareholders/Claimants will have to submit the below mentioned forms :

• Form A – Affidavit for issuance of duplicate securities                 Download
• Form B - Indemnity for issuance of duplicate securities               Download
• Investor Service Request Form – 4 (ISR 4)                               Download

If Market Value More Than 5 Lakhs:
• Copy of FIR including e-FIR/ Police Complaint / court injunction (An overseas securities holder in lieu of these shall be permitted to provide self-declaration of the security certificates lost / misplaced / stolen duly notarized / apostilled / attested by the Indian Consulate / Embassy in their country of residence along with self-attested copies of valid passport and overseas address proof.)
• Advertisement in a widely circulated newspaper

Claimants will have to submit the below mentioned forms :

• ANNEXURE A - Ready Reckoner : Docs. Req. For Transmission Of Securities    Download
• ANNEXURE C – ISR 5            Download
• ANNEXURE D - (Affidavit)       Download
(Individual Affidavits to be given by all the Legal Heirs or Legal heirs named in Succession Certificate/Probate of will/Will/Letter of Administration /Legal Heirship Certificate or its equivalent Certificate/Court Decree.)
• ANNEXURE E - (Indemnity Bond)       Download
(Bond of Indemnity to be furnished Jointly by all legal Heir(s) including the Claimant(s))
• ANNEXURE F - (NOC)       Download
(No Objection Certificate from the Legal Heir(s))
• Joint Holder Transmission Documents       Download
(Only applicable in case there is a joint holder)

Tax Exemptions

Shareholders will have to submit the below mentioned forms to avail tax exemptions :

• Form 15 G - Applicable to any person other than a Company or a Firm
• Form 15 H - Applicable to an Individual above the age of 60 years
